Annual Medical Checkup

Hokkaido University Health Care Center will provide annual medical check-ups.
The annual check-up is free and provided only in April and May.
Students needing a Health Certificate should take all the check-up items.
A Health Certificate might be necessary when you apply for postgraduate program, scholarships and job opportunities.

Preparation:
1. Pick up the “Medical Checkup Set” at your department in advance.
2. Fill out the “Health Inquiry Sheet” in advance and fill the container in the urine test kit with your urine in the morning of your check-up date.
3. Please collect and submit your urine, even if you have your menstrual period. If you have your menstrual period, please write this on the paper bag of the urine test and also in the “Health Inquiry Sheet.”
Results:
The results will be mailed in sealed envelopes to each faculty and ready to be picked up at the front desk.
Each faculty is responsible for how the results are distributed.
