Expanding eligibility for the COVID-19 In-House Vaccination Program

Family Members of the Faculty, Staff, and Students of the University are Now Eligible
To all faculty, staff, and students of Hokkaido University:
The family members of the faculty, staff, and students of Hokkaido University (18 years of age or older) are now eligible for the University’s COVID-19 In-House Vaccination Program available between Friday, April 29 through Sunday, June 12, 2022. Please see below for details.
Appointments are available on a first-come, first-served basis. The number of vaccines available at the University is limited, and the appointments will be closed when the maximum capacity is reached. Early booking is highly recommended for those who wish to be vaccinated.
- Newly Eligible Persons
The family members (18 years of age or older) of the faculty, staff, and students of Hokkaido University (hereinafter referred to as the “University member”) - How to Make an Appointment
Please follow the procedure below.
①Download the Excel file below, 【Form】List of Persons Eligible for Vaccination (Family Members of Faculty, Staff, and Students) and complete the required information. The form must be sent to the email address below by the University member.

②The General Affairs Division of the General Affairs and Planning Department will assign an Appointment Number to each family member listed in the form, and inform the University member of the Appointment Numbers via email.
③Inform the family members who are receiving vaccination of their Appointment Number, and make an appointment via the Vaccination Program Website below.
④After making an appointment, follow the same procedures as the University members.

- What to Bring on the Day of Your Appointment
(1) Vaccination Ticket (includes a Vaccination Ticket combined with a Screening Questionnaire)
(2) Completed Screening Questionnaire
(3) Certificate of Vaccination for the 3rd dose
(4) Documents proving your identity
(5) Documents proving your 1st and 2nd doses of vaccination
(It may be omitted if you have the Vaccination Ticket with the “Vaccination Record for the 1st and 2nd doses” with the dates of vaccination)

Reserve a date and time slot via the Appointment Scheduling System (V-CHAT)*

Be sure to register your email address when making an appointment.
You will receive emails confirming your request and your scheduled appointment, and an appointment reminder two days prior to your scheduled appointment.
